Starting Meaningful Conversations from Patients’ Real Experiences

The work demonstrates the value of a patient journey map, a tool developed from observing an actual patient’s experience of leaving the hospital. Used in the context of a day-long workshop, it aims to coordinate, translate and align a team of disparate members - including patients, caregivers and healthcare professionals - to the patient’s perspective, in order to start meaningful conversations around transitions of care.
